The under-eye is one of the most requested treatment areas and one of the least forgiving. When the fat pad beneath the eye descends and the bone behind it resorbs, a groove forms from the inner corner of the eye across the top of the cheek. That groove casts a shadow, and the shadow is what most patients call a dark circle.

What to expect from tear trough filler

Consultation & preparation

Avoid alcohol for 24 hours and ibuprofen, aspirin, vitamin E and fish oil for 48 hours beforehand where it is safe to stop them, since the under-eye bruises readily. Come with no eye makeup and remove contact lenses before your appointment.

During treatment

What causes the hollow under the eye?

Two changes, usually together. The fat pad that sits over the cheekbone descends with age, and the bone of the eye socket resorbs slightly, deepening the rim. The result is a groove running from the inner corner of the eye outward. Skin over that groove is the thinnest on the face, so any shadow it casts reads strongly.

Is filler always the right answer?

No, and that assessment is the most important part of the appointment. Filler treats hollowness. It does not treat pigmentation, thin crepey skin, or puffiness from fluid retention, and placing it where the problem is not volume produces a worse result than doing nothing. Dr Harriet uses combination treatments here, drawing on dermal fillers, polynucleotides and specialised amino acid, hyaluronic acid and vitamin combinations depending on what is actually driving the appearance.

Why does technique matter so much here?

Because the margin for error is small. The skin is thin enough that product placed too superficially is visible, and the area has a rich blood supply that demands careful anatomical knowledge. Dr Harriet places conservatively and reviews rather than trying to correct everything in one appointment, and she will decline to treat where she thinks the anatomy makes a good result unlikely.

Aftercare

Sleep with your head slightly elevated for the first two nights and avoid rubbing the area. Skip strenuous exercise, alcohol, saunas and steam rooms for 24 hours. Expect swelling for a few days and allow up to a week for any bruising. Judge the result at two weeks.

Will tear trough filler get rid of my dark circles?

It depends on what is causing them. Where the darkness is a shadow cast by a hollow, filler helps a great deal. Where it comes from pigmentation in the skin or from visible blood vessels through thin skin, filler will not help and a different treatment is needed. Dr Harriet assesses which applies to you.

Is tear trough filler safe?

It is one of the more technically demanding areas and should only be treated by an experienced medical injector who understands the anatomy. Dr Harriet is a GMC-registered doctor with ten years in aesthetics and the eye area as a named special interest. Risks are discussed in full at consultation.

Can tear trough filler be dissolved?

Yes. Hyaluronic acid filler in the tear trough can be broken down with a prescription-only enzyme, which is a meaningful safeguard in an area where results need to be right. Dr Harriet offers dissolving as a treatment in its own right, including for under-eye work placed elsewhere.

How long will it last?

Under-eye filler tends to last longer than filler in mobile areas such as the lips, often twelve to eighteen months, because the area moves relatively little. Your own metabolism and the product used both affect this. Dr Harriet will give you a realistic timeframe at consultation.

How much swelling should I expect?

The under-eye bruises and swells more readily than most areas because the skin is thin and well supplied with blood vessels. Expect some swelling for the first few days and allow up to a week for any bruising to resolve. Do not book treatment close to an important event.
